Your First Appointment

We invite you to call our office for an initial consultation. Please bring pertinent dental and medical insurance information with you. We will submit your insurance forms as a courtesy. Bring any x-rays or models your referring dentist has provided you with to assist us in the evaluation. We may need to collect additional radiographs and diagnostic information in order to finalize your evaluation.

Please feel free to bring your spouse, relative or friend to our office, as important decisions are often necessary.

Inital consultations take about an hour, and complex cases may require additional visits for proper diagnosis and treatment plan. Co-diagnosis and agreement between the patient, referring doctor, and implant surgeon is issential to achieve the best result. The inital consultation fee is due at the first visit. We will inform you of estimated treatment fees and assist you to make financial arrangements prior to treatment.
